Lung transplantation (LTx) is a therapeutic option and the only definitive treatment for patients with end stage lung disease. However, the mortality of patients on the waiting list for LTx is still higher than all other types of organ transplants. The main obstacles to the full success of the LTx programs are the following:

  1. Availability of transplantable donor lungs, which remained relatively steady despite the progressive growth of the candidate pool;
  2. Primary Graft Dysfunction (PGD), which is the final result of Ischemia Reperfusion (IR) injury and is associated with poor outcome in the postoperative period.

One of the most remarkable strategy to overcome lung shortage is the normothermic Ex-Vivo Lung Perfusion (EVLP) that has been shown to be a safe and feasible strategy to objectively evaluate and optimize non-ideal lungs. However, 20% of EVLP lungs are still declined for transplantation because of poor physiologic performance.

The aims of the project are:

  1. to better define mechanisms and therapeutic approaches for modulating lung IR injury that negatively affect outcome;
  2. to identify novel biomarkers associated with IR injury and severity of PGD;
  3. to define pharmacokinetics of antimicrobial drugs during EVLP;
  4. to assess the feasibility of EVLP strategy using lung form Donor after Cardiac Death (DCD).
